Dr. Ali Raza respects your privacy. This policy explains how we collect, use, store and disclose your personal and health information, and how you can access or correct it.
We handle personal information in accordance with the Privacy Act 1988 (Cth), including the Australian Privacy Principles, and the Health Records Act 2001 (Vic).
1. About this policy
In this policy, “we”, “us” and “our” refer to the dental practice of Dr. Ali Raza at 1/35 Whitehorse Rd, Deepdene VIC 3101. “You” refers to our patients, prospective patients, dentists enquiring about our courses, and visitors to this website.
“Personal information” and “health information” have the meanings given to them under the Privacy Act 1988 (Cth).
2. Information we collect
We only collect information that is reasonably necessary to provide you with dental care or to respond to your enquiry. This may include your name, date of birth, address, phone number and email; Medicare and private health fund details; emergency contact details; and payment information.
Because we provide health services, we also collect health information such as your medical and dental history, medications and allergies, clinical notes, X-rays, digital scans, clinical photographs and treatment plans.
If you are a dentist enquiring about one of our courses, we may also collect your AHPRA registration details and the name of your practice.
3. How we collect information
We collect most information directly from you: when you complete forms, attend appointments, call or email us, or submit an enquiry through this website. With your consent, we may also collect information from other health practitioners involved in your care, from your health fund, or from a parent or guardian where a patient is under 18.
4. How we use your information
We use your information to assess, plan and provide dental treatment; to communicate with you about appointments, reminders and follow-up care; to process payments, health fund claims and payment plan applications you ask us to make; to respond to enquiries; to administer our courses for dentists; to maintain accurate clinical records; and to meet our legal and professional obligations.
5. Clinical photographs
Photographs and scans form part of your clinical record and help us plan and review your treatment. We will never use images of you in our Smile Gallery, on social media or in any other marketing without your separate written consent, and you may withdraw that consent at any time by contacting us.
6. Disclosure of your information
We may disclose your information to other health practitioners and specialists involved in your care; to dental laboratories that fabricate your restorations; to your health fund or Medicare; to a payment plan or finance provider you choose to use; and to trusted service providers who support our practice, such as clinical software, IT, booking and communication providers, who are required to keep your information confidential.
We may also disclose information where you have consented, where it is required or authorised by law, or where it is necessary to lessen a serious threat to someone’s life, health or safety. We never sell your personal information.
7. Marketing
With your consent, we may contact you with appointment reminders, practice news or information about treatments. You can opt out of marketing communications at any time by using the unsubscribe link or by contacting us.
8. Storage and security
Your records are stored securely, using password-protected systems with restricted access and reputable service providers. We take reasonable steps to protect your information from misuse, interference, loss and unauthorised access.
Health records are kept for the periods required by law. In Victoria, this is generally at least seven years after your last visit or, for patients under 18, until they turn 25. After that, records are securely destroyed or de-identified.
9. Accessing and correcting your information
You can ask to access the personal and health information we hold about you, or ask us to correct it if it is inaccurate, out of date or incomplete. Please make your request in writing to the contact details below. We will respond within 30 days. A reasonable fee may apply for providing copies of records, and access may be refused only in the limited circumstances permitted by law, in which case we will explain why.
10. Website, cookies and analytics
This website may use cookies and analytics tools to understand how visitors use it and to improve its content. This information is generally aggregated and does not identify you. You can disable cookies in your browser settings. Information submitted through our enquiry form is used only to respond to your enquiry.
11. Information stored overseas
Some of our technology providers may store data on servers located outside Australia. Where this occurs, we take reasonable steps to ensure those providers handle your information in a way that is consistent with Australian privacy law.
12. Anonymity
You may make general enquiries without identifying yourself. However, to provide dental treatment safely we need to know who you are.
13. Complaints
If you have a concern about how we have handled your information, please contact us first and we will do our best to resolve it promptly. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or the Health Complaints Commissioner Victoria (hcc.vic.gov.au, 1300 582 113).
